Rental Requirements and Rules: 
 
The following paperwork must be obtained before you can start using the facility: 
 
Health Permit (Santa Clara Dept of Environmental Health 408-918-3400) 
Inform the inspector that you would like to obtain a health permit to operate your business at our facility. 
 
Liability Insurance (you must have an insurance policy of $1,000.000 for your business, and Amy's kitchen rental must be included in the policy as additional insured). You can contact PHP Agency at (408) 528-1528 or any Insurance Agency for quotes. 
 
All your proper paperwork (health permit, food certificate, your contact information) must be displayed in the front office for routine inspections.
